i'm having my root canal next weekend on a top premolar but it's not due to infection. a filling is causing irritated or inflammation of the nerve. is that less serious then an infection? could a inflamed nerve turn into a infection? just curious?

: The filling is deep and close to the nerve[pulp] ,causing pain and discomfort. If untreated this will lead to the death of the nerve and an infection. You will than have to be placed on an antibiotic and lose the tooth if you don't  get a root canal. The root canal is the proper treatment.

is clumping of caudal nerve roots a common incidental finding on mri scans? have had lots of corresponding symptoms. no compression, no back surgery. but have the inflammation of the nerve roots. what can cause caudal inflammation? thanks?

: Do you have a history of spinal injury? You are too young for spinal stenosis. I would expect symptoms affecting your urinary bladder and legs with inflammation of the caudal nerve roots. In view of the MRI findings I recommend consulting a neurologist or a neurosurgeon for evaluation and advice. The specialist may review the MRI on his own or sit down with the radiologist to get more 
 input.

Compression: The mechanism is different, usually through mechanical compression from inflammed structures, which in a sense might sound like the body is attacking itself. A typical example of a disease in which there is damage due to immunity directed to its own nerves is multiple sclerosis and that will be a good example for an immune mediated disease, or body attacking itself.

Epidural steroid injections are sometimes used to place anti-inflammatories into the spine.

: Although not yet widely available, MR imaging of peripheral nerves is useful for persistent nerve-related symptoms despite prior normal imaging and nonspecific EMG results.  Can be helpful with lumbar or brachial plexus issues, thoracic outlet, sciatica.
