See you doctor soon: It may not be a  sebaceous cyst , most likely it is  pilonidal cyst , Infection may cause complications , even in pregnancy infection  ( pus ) can be drained under local anesthesia . Speak to your OB doctor

does pain near tailbone with a pilonidal sinus always indicate that there's an infection? or could the sinus hurt even without being infected? also, i don't feel any painful bump, just the sinus. no pus drainage as far as i can tell?

GET IT CHECKED: AFP Alfa fetoprotein normally is less than 10 ng/dl

High AFP may be seen in Lymphoproliferative Disease and Kidney Neoplasms.
Sometimes in Liver Neoplasms!
Best go to your physician to have it checked out! 
Only high AFP in Hepatitis C Viral Infections with Liver Neoplasms ! Rare otherwise in most infections!

You may have bladder: Orthopedic problems rarely relate to urinary bladder problems.  However, rarely, bladder tumors invade bone in the coccyx, or "tailbone."  you need x-rays and probably an MRI to evaluate.

my wife has a lump on her coccyx which she broke 20 yrs ago. it is painful for her sitting and standing up. it isn't red and doesn't look like infection. sometimes it even seems to soften up and move a little. what could this be?

Scar tissue: This could be scar tissue, reaction to the fracture. Not sure if it is amenable to  "shaving" to reduce it. An orthopedic surgeon could help evaluate that.

Not to my knowledge!: Some antibiotics can cause diarrhea with soreness around the backend, but specifically affecting the tailbone is unknown to me.
