Hello. : Hello. A sed rate is another term for an erythrocyte sedimentation rate (esr).   It is a test  that can help detect inflammation (acute or chronic).  However, it is a nonspecific test, so the ESR could be elevated for many different reasons to include infections, cancers, and autoimmune  diseases. Your doctor will no doubt use this piece of information combined with results of other lab tests, your medical history and your physical examination to help determine the cause of your symptoms.

See a rheumatologist: The vitamin D level is unrelated. The mild elevation of the ESR may be consistent with your symptoms. Have it evaluated, preferably by a rheumatologist. It may be an inflammatory arthritis such as RA or others.

That's good: I would urge you to draw no conclusion from any sed rate reading that's not extremely high unless there's specific symptoms and signs.  Millions of healthy young adults have positive ANA at a titer of 1:40 speckled. I'd urge you to get into a fitness-focused lifestyle, hang out with quality friends and do meaningful things. Not a problem : There is no disease associated with a low ESR.   An elevated ESR is a nonspecific indicator of inflammation. There are many possible of a high ESR--infection, malignancy, or autoimmune disease.

Autoimmune disease: Autoimmune disease like lupus/sle is in the top of my differential diagnosis. Go  and discuss with your pcp. Your pcp can refer you to see a rheumatologist as indicated.

fatigue
generalized darkening of skin color (often referred to as bronzing)

Possibly: Several diseases are characterized by both arthritis and neurologic issues. Have a rheumatologist evaluated the issue.

Numbness/tingling: I would also order crp, vitamin, B12 level and hba1c to evaluate multiple causes of neuropathy. Vitamin B12 of less than 400 would need rx with non oral supplements.

Make diagnosis: Those are a lot of things and they may or may not all be connected.  Certainly with an elevated sed rate and a positiveana test are concerning for  an autoimmune condition, such as lupus, would be a concern.  I would encourage you to continue work up with a rheumatologist and ultimately the single or multiple diagnosis made will aid in treatment and prognosis discussions.  Thank you.
