Yes and no: Image guided sinus surgery allows your surgeon to more accurately identify where they are working in an effort to reduce the chance of surgical injury and to assure that the sinus issues you suffer from are treated as effectively as possible. This does require additional radiation exposure for the scans, something that one should keep to a minimum. For many, benefits outweigh the risk.

Depends: For a first sinus surgery image guidance not necessary unless your surgeon feels that severe anatomic distortion exists or mucocele impinging on important structures.  Since mucocele is expansive then it has done most of the work and it is usually easy to open and drain it for an experienced sinus surgeon. Ask how many surgeon has done or get second opinion.
