Dettol soap on face : Dettol soap is not formulated for use on the face, although like you, a lot of people use it for facial cleansing. It may have caused inflammation and subsequent drying and post- inflammatory hypopigmentation. I would stop using the soap and opt for a milder facial cleanser appropriate for your skin type & skin concerns. Also use a moisturizer to help restore hydration to your skin. Add spf in am.

is it safe to use maderma scar lightening gel to lighten the dark skin around the anus? i have so much left over and don't have any scars to put it on

Probably: It may be ok but not sure what your goal is. This will get dark again. There are places that would do that blanching and probably last longer. Is this something that just happened or all your life. Let me know. Thanks

Fade Acne Scars and: It i supposed to fade acne scars and does skin resurfacing.It may also improve pigmented lesions on your face.
I may not make your skin lighter, if that's what your question is, but it will fade pigmented lesions
consult a dermatologist who offers this type of laser therapy.

Needs HIV test: It's impossible to say without examining the patient, and there are certainly a lot of reasons other than aids to have dark spots on the body.  That being said, with a known exposure to hiv, it is crucial that both of you get tested. Please take care i yourselves and get checked asap.

Your menstrual pattern and any birth control are factors. You may also benefit from oral spironolactone. The topicals may be irritating at first and they require 2 to 3 months to show effects.

Stop self-injuring !:  Tooth paste wasn't designed for use on the penis.  Scrubbing vigorously can cause damage. At this point, I can only encourage you to stop the self-injury.  Depending on severity, you may wish to see an urologist.
