Even after your growth plates close if you don’t take care of the origin of your problem you will continue to have it. It’ll just manifest itself differently as opposed to being a growth plate problem. There are different causes for this problem anywhere from a limb length difference to pelvic rotation

Ilium or ileum?: This is a little confusing. The ilium is part of the bony structure of the pelvis. The ileum is part of the intestines. From the other part of your question I suspect you mean the ileum.  If so then this pain could be caused from constipation. It could however be something more serious. I would try to relieve the constipation and if the pain does not resolve then I would seek medical care ASAP.

Yes, possibly: Your ilium is the large wing-shaped pelvic bone. The medial portion of your ilium is the part closer to your spine.  A lesion in your ilium could cause pain in your back and buttock area, but so could many other things  including a compressed nerve (from a bulging disc), a tight piriformis muscle,  or inflammation in your sacroiliac joint. It sounds like your pain "refers", making me think nerve.

Apophysis-sartorious: Rest, ice, and decreased activity is the usual treatment of choice for injury to the iliac crest. The growth plate is likely just about to close via the apophysis. This is where several muscle take their origin. In your son's case, it is likely an apophyseal fracture, minimally displaced, and amenable to nonoperative treatment

Lytic lesion: This is a lesion in which the bone is being destroyed.  This is typical for cancers that spread to bone. Such cancers include multiple myeloma, breast, thyroid, lung and kidney.  There are a few reasons to seek MRI - best to check with your doc as I would simply be speculating.

Steroids?: have you seen your doctor to give you medicine for this episode?  See your GI.  Try drinking water or Gatorade.  Fiber powder to slow diarrhea down.
