If growing bigger: If one testicle is growing bigger than the other one, after puberty, there could be a tumor in the growing testicle. A doctor's evaluation is needed. A ultrasound scan can be done. If a child developed one small and one bigger testicle, but they stayed the same size after puberty, they are likely ok. However, if the smaller one then grows to match the bigger one, a doctor can check for cancer.

Rule out tumor: Need to rule out testicular tumor, quite possibly a sertoli or a leydig cell tumor. Boy needs to have tumor markers of alphafetoprotein (AFP) ; beta hcg. Unusual for it to have grown so large. Also needs a scrotal ultrasound. Tumor needs to be removed with intra-operative frozen section pathology if available. This is a rare tumor ; hopefully benign. Needs expert care. Please keep us informed.

Enlarged testicle: The most worrisome condition would be a malignancy (cancer). It could also be a collection of fluid (hydroceole/spermatoceole/epididymal cyst).  Regardless, you should be seen by a urologist sooner rather than later.

Testicle Anatomy: It sound pretty normal to me.  One testicle is the dominant one and one is the backup in case of injury to the dominant one.  Healthy testicles are almond shaped.

No: If this has indeed happened, you should be examined right away. All adult makes should be performing routine monthly self testicular exams.

See remark: There is a possibilty you had MUMPS as ayoungster &amp;
it causes atrophy of one testis.
Or  you may  have had a sporting injury affecting the scrotum.If in doubt consult with a urologist.

There is a possibility you have a hydrocele (extra fluid
around the testicle )

See a urologist: 36 years of enlargement is not likely a serious problem. One common cause is hydrocele.  This condition is due to fluid around the testis and can be surgically treated by a urologist.  Get examined

can a very small varicoele cause discomfort when standing for long periods of time? i was told only big ones do, but i always get that heavy feeling on one testicle if i stand for a few hours. i have a grade one varicocele. does this make sense/ normal?

Dull ache: can occur;however, if not performed recently, get a scrotal ultrasound  and a urology consultation

my right testicles is still big after childhood mumps.i am 22 now, feel no pain but left one shrunk a little bit.should i worried about infertility?

Unlikely an issue: A sperm analysis should be able to answer your personal question about possible fertility.Mumps is more likely to cause testicular issues if you have it in your late teens or adulthood. That said, it is not common for it to cause infertility.You have 2, and if one functions well enough, it could do the deed.
