Asthma : Definitely visit your allergist or primary care provider for additional options. It's important to find out if allergies are triggering these symptoms. An allergist might also consider you for Omalizumab injections. You should go to the ER if the symptoms aren't getting better and if you are still experiencing shortness of breath with chest tightness

Mgt: There are similar medications and alternative medications available. I would suggest discussing Breo with your pulmonary specialist. 
Did you have a lab evaluation including muscle enzymes and electrolytes? I would suggest further investigation for these findings.

Likely safe: I am not familiar with the product omega XL..I suspect it has to do with omega 3 supplementation.   I wouldn't expect any clinically significant interactions with omega 3 fatty acids and the above regimen.

a dr. prescribed levofloxacin , 500, mg, i stopped taking  after 5 days, that was two days ago, my kidneys are hurting both side i have copd i take spiriva or advair for that, and had a lung infection, he told me he would prescribe some strong meds. i ask

By : By the date of this post a week has passed. I hope your issue has been resolved. If not you need to make contact with your doctor right away. If you ever feel "really worried" you must seek care right away. You could have had an adverse reaction from the Levofloxacin or even a urinary tract infection.

There : There is always help if you are ready to receive it, internalize it, and act on it. Due to your multiple problems you will need a multi-disciplinary approach. Start by following up with all your doctors and following their recommendations.
The key here i believe is for you to seek counseling. Dealing with so many issues is very difficult and can take a toll on your mental well being and your ability to cope and deal with what may seem to be an overwhelming or hopeless situation, and to ultimately accept all possible outcomes, good or bad. 
In addition you need to be screened for depression and treated accordingly.

Probably: Usually we don't change medication when it has worked well on a patient.  Given the circumstance, Incruise would be a reasonable substitute but do realize that they are similar but not identical.

Effective drug: Spiriva is not necessarily the "best" because it is usual part of the regimen for many patients with copd. Spiriva has been shown to be very good at relieving a feature of COPD called "air trapping". That is patients on spirva can be shown to on average trap less air in the lung on pulmonary function testing. Air trapping correlates with shortness of breath, so this makes spiriva effective.

is spiriva a form of steroids?

Accepted Answer

No: Spiriva is in a class of medications termed anti-cholinergics.  These medications when inhaled induce some bronchodilation as well as some "drying" of the airways.  It is indicated for COPD and can help the productive cough and wheeze.
