Mgt: If the pain only occurs with a cough, most likely the pain is caused by muscular-skeletal pain. I would suggest a heating pad and Motrin. If it continues, or is associated with respiratory difficulty, a follow up appt is indicated.

Hello. : Hello. You might talk to your doctor about tessalon perrles or use of codiene. Both can be very helpful with continual coughing. Make sure that you hydrate really well. At least 8 8 ounces glasses of water per day.

Probably not.: I think it is just coincidence.  If anything, your breathing should improve when you stop smoking.

Maybe Viral: The chest infection may be viral and will not respond to any antibiotic. Clarithromycin is a good broad spectrum antibiotic for a chest infection. Get some Mucinex (guaifenesin) DM to help with your symptoms. I hope you feel better soon.

Infection usually: Any infection that causes irritation to the respiratory system will cause a productive cough with sputum. It is your body responding to the infection. Unless you are running fevers or it has been going on for more than two weeks you can probably just wait it out for it to heal.If it has lasted more than two weeks then see your dr. Or if you are running fevers have shortness of breath or pain.

: As long as the kid eats well, plays and remains fever free I would not be concerned. Most of these are self resolving viruses. If feeds are off as are other daily activities I would have him checked.

Musculoskeletal...: Pain as you describe is likely musculoskeletal pain related to coughing. The chest wall has muscles, cartilage and bone and these structures can be injured when coughing. For example, muscle strain/sprain, cartilage inflammation, rib bruise or even fracture can occur with coughing. When the cough resolves, this will heal so see your doctor for cough treatment.

Possible causes: There are many possible causes, and bronchitis, sinusitis and may cause prolonged cough. Nasal saline, hydration, monitor for fever. Please see your physician for further evaluation and testing.

Cough in winter: If you get these symptoms only during the winter months it may be triggered by allergies and would recommend you have allergy testing done to see it is something you can avoid or get treatment for.  I would also recommend you get your flu shot in late September annually.
