Firstly, : Firstly, there is no reason to be scared if you have a cavity... You are not falling apart.

Forget about diagnosing this by yourself with a needle. You are correct: it is either a stain, or a cavity. A dentist, examining your tooth physically, can tell you for sure.

Staining is caused by bacteria that gets caught in the pits and fissures of your teeth, or from the actual coloring of the food you eat.

Make an appointment, get an exam for all of your teeth, and make sure that you don't have any other problems that you cannot see.

Floater, no problem: spots like this are usually the result of one or a few red blood cells getting loose from a capillary in the back of the eye. They may not show on exams, but because of a magnifier effect, may seem large to you. Most likely nothing to worry about. If you are on blood thinners and they get worse this should definitely be reported.
