Cartilage or rib: You have either stretched the intercostal cartilage between the ribs or broken a rib. You need to see a MD and get a chest xray.

Pleurisy: this type of pain with breathing is usually caused by a muscle strain between the ribs tho sometimes a rib can get slightly "out of place" - they usually pop back in hours to days. If severe, or lasting longer than a couple days, or if you feel short of breath now, see a doctor today. Otherwise, some docs and phys therapists can offer manipulation that can help this.

was skating n i ended up hitting a disk on my back between shoulder blades.bruise went away but still hurts a week later. sometimes hurts to breath,flex arm n get up. disk feels swollen and tender when i touch it. sharp pain on the right side of disk?

See below: You may have injured one of the bones in your back or even a rib.  The discs are much deeper than just below the skin and you can't actually feel them.  See your physician for further workup.  Sound like you might need an x-ray.

Shoulder pain: Pleurisy  typically hurts more with breathing.  Pain with shoulder movement is more likely related to the muscles of the shoulder blade.

Unlikely: Shortness of breath on exertion with chest pains sounds more like a cardiac condition or copd.  Pneumonia is characterized by coughing, sputum production, fever and sob at rest.  Pleurisy causes chest pain on inspiration.  I suggest you see your md immediately and get checked out.
