Nothing particular: Hypothyroidism often requires levothyroxine.  Alcohol does not alter the requirement or interfere with the replacement.

Portable Breath Test: By PBT, I assume you mean Portable Breath Test aka Breathalyzer. General consensus is that we take an hour to clear a unit of alcohol which varies depending upon type (and amount) of drink. Neither: It does not really make a difference. Alcohol in large doses is a mitochondrial poison, and would tend to adversely affect you.

elevated tsh, normal t4, no hypothyroid symptoms. have recently quit caffeine / significantly reduced alcohol (in case it's adrenal fatigue). is this likely to lower tsh? how long to see results?

There is: There is a condition called subclinical hypothyroidism where where the TSH can be slightly elevated and a patient may or may not have symptoms.  If the patient does not have symptoms, medication is usually not started and the TSH is rechecked later.

i have some abdominal discomfort for the last couple of hours. i did consume a moderate amount of alcohol last night. i am on medication for depression, cholesterol, and hypothyroidism. what could this be?

This could be any: of a number of different things. If it persists for more than 36 hours go and see a health care provider for evaluation.

It is possible: If you are concerned about hypothyroidism and have some of the following symptoms, 
feeling tired, weak, or depressed, dry skin and brittle nails, hard stools or constipation, fatigue or feeling slowed down, you should be evaluated by your primary care physician with a simple blood test to check your thyroid hormone levels.

Thyroid: Feeling hot and cold can be a symptoms of inflammation  of the thyroid.  This may be non-specific thyroiditis or Autoimmune like Hashimoto's. Traditional medical care focuses only on TSH and the inactive thyroid hormone called T4. A detailed thyroid panel is not done as the management does not change. However in functional medicine we focus on thyroid and adrenals. You may need this.

Both results in the: Abnromal range but not by too much. In any case the levels are indicative of thyroid deficiency and could lead to your symptoms. Please see your doctor for replacement therapy.

Gold standart: The gold standard for diagnosis of celiac disease is small bowel biopsy. Of course all clinical information is important. I do not think it is any risk if you are going to try gluten free diet. Please, consider consultation with gastroenterologist.
