When the left atrium becomes enlarged, the portion of the left atrium called the left atrial appendage, which forms the left heart border, enlarges, thereby causing the contour of the left heart border to become convex (bulged out).  Atrialization refers to this change.

Difficult to say: You are right, the heart is located(usually) in the left side of your chest and can present with left sided pain when something is wrong.  But many other things can do this too.  Issues with your lungs or your stomach or spleen or muscles in the area can present like this.  Even stress/anxiety can give you pains like this too. Normal variant: What your brother has is something called dextrocardia and is a known variant or mutation that is usually benign. He was born with this diffrerence. Some people are born with all of their organs reversed to the opposite side of where their organs should be. Again, it is normally not something to be worried about just an interesting medical curiosity.
