Itching: skin itches for many reasons including bacterial contamination , moisture, fungal infections, tight clothing, sweat,  contact dermatitis, etc.  Most genital areas itch when they get moist or hot.  It is normal to have itchy skin in this area.

Scrotal pain: Scrotal pains can be from many types of issues. This could be a testicular issue, a hernia or referred pain from nerve irritation somewhere in the abdomen. I would recommend seeing your primary care doctor who can do a physical exam and begin a work-up and treatment plan.

If swelling, or pain: Scrotum or testicle swelling or pain means a trip to the doctor or ER because a testicle's blood supply can be twisted off (torsion). When that happens, the testicle can die. Since one usually cannot tell for sure if his testicle is twisted, he needs an exam by a doctor. That said, if the swelling is only the scrotum skin, or due to a mass inside the scrotum, the doctor can help those issues too.

Need more info: Need more info. Do you feel one small testis on the left?  Do you feel a normal testis and an additional small "ball"?  If two "balls" on left side are they connected or do they feel separate?  This could be just a small testis, spermatocele, epididymal cyst, varicocele, or an abnormal growth.  Either way see your urologist and have an examination and likely a scrotal ultrasound to sort this out.

Cortisone: OTC 1 % hydrocortisone can be used on the scrotum safely in most instances however do  not apply cortisone to any genital area without medical consultation. The skin of the scrotum is quite thin, and overuse can cause an infection to an already irritated moist area of the body

Many things.: Acute scrotal pain can be caused by testicular torsion, infections such as epididymitis, orchitis or mumps, appendix testis torsion, cancer, hernia, hydrocele, spermatocele,  varicocele & traumatic injury.  If you have sudden pain, please see a physician since torsion or infection must be treated immediately. A physician will be able to assess the need for scrotal ultrasound & direct treatment.
