Need cause: Hydronephrosis just doesn't happen - there is usually a cause that can identified. Is there urine reflux, are there posterior urethral valves, is there an intrinsic problem with the kidneys. Once the cause is identified, then you can focus on treatment. Need workup by pediatric urologist and/or pediatric nephrologist before kidney damage occurs.

Not to worry: Your baby will have an ultrasound after birth to see if it is still there.This is a finding I read a lot on prenatal ultrasound reports.  So far none in my experience have had it after birth even though they were all thought to have it prenatally. 
It means the kidney on the affected side has developed an appearance suggestive of back up fluid and pressure on it as if something is blocking the flow of urine from the kidney to the bladder.

Wait: Half of prenatally discovered hydronephrosis will not be present after delivery. The ultrasound should be repeated on the child at that time. The remainder of the children with persistent hydronephrosis may need further workup, but the majority of these problems can be corrected or watched.

It depends.: Much controversy exists re: the assessment & management of hydronephrosis in infants. Asymptomatic  hydronephrosis of any grade, without urinary infection & normal renal function can be managed conservatively. The goal is to select  those infants with severe obstructive dilatation during serial follow-ups and to perform surgical repair before irreversible deterioration & renal damage occurs.

As long as necessary: The treatment plan for tour child is likely a complex one that involves testing & consulting with pediatric urologists/nephrolgists & your primary pcp.You do not mention the main diagnosis, but there are situations where intermittent cathing is done indefinitely. The option of a continuous catheter may lead to frequent urinary tract infections.

Do nothing now: Baby is almost certainly perfectly OK. If renal pelvis 7.5mm, nothing to worry about &amp; will 99 - 100% settle down as a normal kidney. 7.5 cm is very large &amp; will almost certainly require intervention. Will require:- pediatric urologist, post natal US , VCUG &amp; nuclear scan after birth to evaluate swollen kidney for function. Removal only if no or essentially no function. Life with 1 kidney is OK.

Retained urine: Results in kidney swelling, most commonly due to obstruction between the kidney pelvis (urine funnel) and ureter which drains urine into bladder (uretero-pelvis junction obstruction), less commonly due to obstruction at bladder end of ureter or bladder reflux, in both cases ureter will will also be swollen. Unilateral means that hydronephrosis is only present on one side.
