Unlikely: Simple topical use of mild steroid creams will cause problems with  fetal development. Ingestion of any medication would be a different matter as it enters the circulation. Any ingested med would have its own safety profile for a fetus.

Not directly: Cortef (hydrocortisone) is similar to cortical adrenal hormones. The placenta is able to break down maternal cortical adrenal hormones, so that the fetus is not exposed to an excess of them. However Cortef (hydrocortisone) (like any other cortical adrenal hormone) can cause hypertension and diabetes in the mother, and these may negatively affect the pregnancy. As always, one has to balance risks and benefits...

Steroid shots: Steroid injections like Dexamethasone and Betamethasone are given to pregnant women at risk of preterm delivery to help the babies lung mature faster. It is beneficial to give steroids in these circumstances. Cortisone is a different steroid and is not used for this purpose. If your doctor thinks that it is an appropriate use for your shoulder then it can be given.

Risk versus benefit: We would ideally like to avoid all medications in people who are pregnant. If the benefit is necessary, corticosteroids can be used in people. This can be done safely in people but interestingly not in certain animals. If you need to take corticosteroids then it is safe with pregnancy. We always are cautious with the use of corticosteroids in anybody pregnant or otherwise.

May not be safe: Adequate studies are not done. However, cortisone crosses the placenta and there may be an association between cleft palate in infants and first trimester use of systemic corticosteroid. Also, there may be fetal growth retardation. Given, the short duration of pregnancy, I would refrain from using Cortisone injections until after pregnancy.

Accepted Answer

Pregnancy ?: Any amount of semen near the vagina can contain sperm, and pregnancy is possible. If this occurred just after your period, and before you ovulated this month, the likelihood of pregnancy is low.  Cortisone will not delay or affect the period.  If your period is "late", get a serum HCG test to be certain you are not pregnant.
