No: No evidence that it affects fertility. In any case hydroceles in adults are almost invariably one sided so other side would not be affected. Hydroceles, unlike varicoceles do not affect testicular temperature & thus don not affect spermatogenesis.

Depends: Hydroceles can become very large and even painful/problematic.  If it is just a small hydrocele, you could safely watch it for a while.  Infrequently it can interfere with fertility.  If it is large and bothersome, I'd have it removed.

Rule out STD: Probably should have your urine tested, plus any urethral discharge if present. Which also means that you should see a dr.

Yes: Yes I recommend that you get the surgery done. It is not a major surgery and  can be done as outpatient durgery and recovery is fast too. You have a choice. Why would you like to live with hydrocele at age of. 21

The positions: of the testes are often somewhat variable within the scrotum, and they may lie along different axes.  If you are not experiencing any other symptoms, the position of your testes may be normal for you.  Talk to your doc if you have concerns.

Check sperm count: If sperm count is OK you are fertile and no problems to have children.  As you have hydrocele surgery on right the fixes the testicle to scotum  and prevents torsion in future.

Wife missed period: Have your wife do a pregnancy test, you may still be fertile despite a hydrocele.
