Sinus infection: Before you start any antibiotucs you shoud be checked by your MD.  A Zpak canbe used to treat a sinus infection if your MD feel itrs the best for you.

Don't worry: your symptoms are unlikely related to corvid 19, and rather to change of season and smoking cessation
try to use a saline nasal spray and Otrivin nasal spray at least 4-5 times a day 
don't use Otrivin longer than 10 days

taking 400 plaquenil (hydroxychloroquine). homogeneous  ana, ch50, compliment 4 still elevated. inflamed sinuses, fatigue, pericarditis ,rashes.  what illness indicated?

Lupus: Systemic lupus is most likely although chronic sinusitis is a component of Wegeners. If all of these issues are active then Plaquenil (hydroxychloroquine) is not effective as the only therapy. Options need to be discussed with your rheumatologist.

Sinusitis: MRI is not a good imaging modality for sinusitis as most normal sinus lining may appear "inflamed" to a radiologist. You may have mild allergy. You likely were imaged with MRI for facial pain and this is an incidental finding. Sinus X-ray or CT would be more appropriate along with an ENT exam to identify OMC obstruction and abscess.Sinusitis is treatable depending on cause.

Depends: On the extent of sinusitis and whether there are other issues, like bone erosion or other structures affected in the same area. Your ENT would decide based on history, physical exam, lab results and imaging studies. Better to get the input of an allergist/immunologist as well, if feasible, best wishes

No effect: Masturbation should have not effect on one with sinusitis.  If you are one who gets nasal stuffiness with sexual excitement, then one could have a temporary issue with the sinusitis.  Treat the sinusitis as usual and masturbate as you feel the need and as you are comfortable.

Stop smoking: The obvious step is quitting smoking which is a common cause for chronic bronchitis (emphysema as well). The next issue is whether you need an antibiotic for your sinusitis which can cause your cough.
