Heals on it's own: A fever blister is a virus (herpes simplex)  nystatin is for fungus.  Ice applied to the area will help with the pain, as will something like benzyl alcohol.  If you get many of them or frequent outbreaks a doctor can prescribe an antiviral. This may also reduce the time you have it by a day or so.

1) it is small, near surface, viral, short duration
2) virus is not very likely to spread or cause other sites of infection BUT. getting realy fresh exudate and spreading it in open wound or eye is one way to cause the worst outcome and if let alone the virus dies out by the time the blister pops on its own (don't get it in your eye in any case).

I : I would question whether this is a fever blister given that it has been present for 2 months. Perhaps it is a condition called perleche?  I would recommend you see a dermatologist who can examine the area, possible perform a bacterial or viral culture, and give a recommendation.  I hope that helps.
