Huh?: You'll need to describe your problem more clearly.  Blood clots occur in large veins, which are not found on the face.  Do you mean a bruise, or something else? A picture would be useful.

: Do not pick at it or squeeze it any more! Dab it of with a warm moist cloth and cover it with a skin colored bandaid (Elastoplast) . Spider veins or haemagiomas do not get better by picking at them and they heal slowly.  Please see your skin doctor!

Warfarin alternatives are apixaban and riveroxaban. Their drawback is they are not as easily reversed if a bleeding problem occurs.

No good options: An old blood clot is difficult to remove, unlike a fresh clot. By now it will have organized & stuck to the vessel so cannot be removed. However taking anti/inflammatory medication such as Aspirin and blood thinners & wearing compression stockings will help it slowly get better.

Heated needle: If I am understanding you correctly, you have a blood clot underneath your fingernail (which can be extremely painful). Draining the blood can provide relief and the easiest way to do this at home is to heat a needle to sterilize it and gently make a hole in your fingernail to allow the blood to drain (a very hot needle also makes it easier to puncture the nail). Otherwise, see a doc for help.

Does it hurt?: If it hurts it can be drained. I would recommend you see a dermatologist to do that. If it is not painful, as the nail grows the clot will grow with the nail as it sticks to the underside and when you cut your nails the clot will eventually reach the end of the nail and you will end up cutting it away.
