A little too much: Avg wt gain is 25-35 lbs, and you should not be gaining more that 1 pound a week at the max, talk to your doctor about your weight, i always discuss that with my pregnant patients as your doc should be doing with you.

Go see doc: If you are eating the same amount and types of food as well as exercising the same,you need to see primary and get some labs checked especially thyroid.Another thing are you drinking soft drinks or sweet tea etc -a 12 ounce soft drink every day for 1 year will put on the average person 10-15 pounds.Small changes in diet can make big differences

Something seems amis: While I trust your weight may fluctuate a bit, it seems like you are reporting very large fluctuations.  Did you lose 30 lbs intentionally, which is possible?  And how fast did you gain the 40 back?  You wouldn't be able to gain 40 lbs of mass without it really only being fluid retention in any short period of time.  We'd need more clarification to better guide you.

Caloric supplements: Try adding carnation instant breakfast to whole milk. Other options include boost, ensure or the "plus" version of either. Eating energy dense healthy foods such as nuts or nut butters will also help. Aim for 2-4 lbs per month as reasonable goal.

See your doc: There is an acute change with your condition and it should be evaluated.  Get your thyroid checked.  Check your blood sugar and see what it's doing.  If it's unchanged, then there is something else going on.  Janumet (sitagliptin and metformin) should not cause constipation or weight gain.

Consume calories: It is possible to gain more weight by consuming more calories then you expend. What you want to be careful about is gaining the right type of weight. You would typically want to gain muscle weight, more than just fat. Please consult your pcp to make sure you have no underlying cause for low weight and he/she will be able to guide you to a proper exercise program as well.

Eat healthy fats!: If you're trying to gain weight, do it in a healthy fashion.  Don't eat junk, but rather add healthy fats to your diet.  Drizzle a teaspoon (or two) of olive oil on your veggies or salad.  Add avocado to everything!  snack on nuts.  Put peanut butter on toast or just have a spoon full of natural pb as a snack.  Also try to add some muscle mass by strength training.  Muscle is healthy weight.

Weight: I would go to GNC or the like and purchase a weight gain product. These are packed with calories and protein so be sure to drink extra water during the day. When you reach the weight you want cut back on the product, eat normally and find the amount you need to maintain the wt. I know many people who would love your problem, good luck.

Probably not: Your body mass index is dangerously low. You absolutely must see a doctor as soon as possible, and pregnancy should not be at the top of your concerns until you are much healthier. Good luck.
