Very short time: Levamisole would be gone in a couple of days, but if you are using Cocaine regularly that is cut with levamisole it can cause a great deal of damage, both reducing the number of white blood cells, causing infections (esp severe mouth sores) and can cause an inflammatory disorder which can cause damage to skin (esp ears, nose cartilage) and even vital organs (vasculitis).

Why are you asking? : In the us ; canada, ketrax (levamisole) has two uses: deworming livestock ; as an adulterant for cocaine. It was removed from the us market in 2000 ; from the canadian market in 2003 due to toxicity to include agranulocytosis. It is not safe during pregnancy.

Yes: Levamisole is used to deworm  pigs, cattle and sheep.  It has been used to cut cocaine.  A number of cases of bone marrow suppression and rotting skin due to an immune reaction have been associated with Cocaine cut with levamisole.

Worm medicine: levamisole was used to treat certain helminthic (worm) diseases in humans, but was pulled from the human medicine market because of serious side effects, and now is rarely used even in veterinary practice because of the same kinds of side effects - very low white counts, severe and serious infections and inflammatory/autoimmune disorders. It is being used to "cut" cocaine for unknown reasons.

Add corticosteroids: Given your genetic history, it is often important to know the underlying genetic alteration as it helps determine treatment effectiveness.  Immune system modifers like levamisole have shown to be very effective.  There are studies which show even more improvement with either topical corticosteroids or oral corticosteroids in addition to the levamisole. Also avoid proton pump inhibitors.

Tramadol: The maximum daily dose of tramadol is 300mg.  Do be cautious, however,  tramadol is officially not considered to be addictive, but my own experience with it in patients has lead me to believe that it can be.

In general: Drs who perform esi's will only do 3 per year (and this is also what insurance will generally cover).  These can be spaced out every couple of weeks or over several months depending on response.  If someone gets 3 in a row in a short period of time and there is little or no relief, esi is/was probably not the right solution for that patient.  These work best for pain from acute nerve compression.

how often can you take a methylprednisone dose pak for gout flare ups. i've been taking them off and on when ever i get a fare up which is about every 4 to 6 months, sometimes longer?

: That amount of spacing shouldn't pose a problem from the medrol dospak. Are you taking allopurinol regularly to reduce uric acid level and prevent gout attacks?
