Very expensive: Liver transplant surgery and meds to prevent rejection are very expensive. However, most insurances will cover that cost if they deem it appropriate.

Inconsistent: The liver can partially regenerated if a portion of a healthy liver is removed surgically.  When you take drugs that cause liver toxicity, they do so over the entire liver and may cause damage severe enough that regeneration is not possible.

Really depends on : Size, number, location.  Even if these are benign, if they get large enough, they can cause mechanical problems or turn into cancer.  Ask your liver specialist for specific recommendation.

Fatty liver risks: Fatty liver is incredibly common, predominantly due to: high alcohol intake, diabetes, high cholesterol, obesity. The more of these risks you have, the greater the chance of fatty liver. Addressing these risks is an important step to correcting fatty liver. Distinguish hepatosteatosis (fatty liver) from NASH/steatohepatitis (fatty liver + abnormal liver tests that risks cirrhosis &amp; liver cancer).

Different insurance: I would suggest applying to a different insurance company. First, there are many disease that may occur particularly from the liver and to exclude it could be quite problematic.  Second, that's illegal to exclude pre-existing conditions according to current healthcare law. Your friend could also wait until next week and see if the new health exchanges give a better offer (don't know until you try).

Not so simple: Some scarring in liver might be partially reversible, depending on underlying cause. Lots of causes, includong alcohol; infections (usually viral);  toxic effects of drugs, herbs; autoimmune; metabolic, including mineral abnormalities, diabetes, obesity; other. Depends on extent of damage as well. Avoiding causative or noxious agents, like alcohol, may help prevent progression of damage

This means cancer: A malignant neoplasm of the liver means cancer.  What it doesn't tell you is if it originates from the liver, or whether it started in another organ and spread to the liver.  This makes a huge difference in how the liver tumor is treated.  Generally cancer that has spread to other organs is a difficult situation, but there are some examples where we have excellent therapies for these cancers.

Transplant: No.  The only way to prevent a transplanted kidney from being rejected is anti-rejection medications.  The body has a tendency to make antibodies to foreign objects inside it.  Thats how we fight off bacteria and viruses.  Its the same way with a transplanted organ.  Thats why we use medications to suppress the immune system so the body does not reject a transplanted kidney.
