Spiriva : Spiriva handihaler's typical use is to inhale contents of one capsule once a day.
You need to get a firm diagnosis of copd. If not done already you can have pulmonary function tests done to help make the diagnosis. A referral to a pulmonologist may be in order.

COPD: COPD is a chronic lung condition characterized by shortness of breath and cough.  The inhalers help with symptoms but don't change the underlying damage to the lung.  Hopefully, if you smoked, you have been able to quit. That is the best thing you can do for your lungs.

i have copd. i saw my fam dr 4 times in april & have to go back wed. i was on prednison 2 inhalers & 3 diff antibiotic. should i see a pulmonary dr?

Reasonable: A helpful primary care physician will be happy to refer you to a Pulmonologist should you make the request.  S/he may already be considering it.  Lung testing is certainly in order to exclude other potential diagnoses.  Good luck.

No: You have something going on. It could be bronchiectasis, chronic bronchitis, cystic fibrosis, pneumocystis, medications, esophageal reflux, post nasal drip, .... Etc.

It : It is very important that you use spiriva as directed by your primary care provider. Spiriva is usually administered every 30 days from the pharmacy. It would be good to connect with your doctor to advise him/her that the inhaler is less effective and allow him/her to determine a better treatment plan.
Do talk to your doctor before increasing your intake of spiriva.

If you are severely short of breath or coughing uncontrollably, you may want to go to an er. In the meantime, if you smoke, you should definitely stop smoking.

Steroids: The  ability of the inhalers to work depend mostly on the degree or amount of lung disease. If you have significant lung involvement, the inhalers will only relieve symptoms and not reverse any lung damage.

It is safe: It is safe to use an antihistamine with the inhaled steroid. Occasional use of a rescue inhaler is safe. If you are needing it on a more frequent basis then check with your doctor. Allergy symptoms often times respond to Benadryl (diphenhydramine) which may help your lung condition also. Check with your doctor if you have any concerns.
