Balanced approach: Eating less may result in body storing more nutrients.  Frequent small meals, if no complicating medical conditions, often better, as you become hungry.  With increased moderate activity will likely be more hungry and can adjust meal size.  Combination of protein, carbs, fat etc. Also important to get necessary nutrition.  Can also vary diet to determine what works best.  One diet doesn't fit all.

Probably, but why?: For a woman in her twenties, 138 lbs sounds normal (but that depends on her height, and what she has been weighing over the past 12 months). In general, we doctors like to do things the natural way, meaning we prefer weight gain by eating nutritious foods that are part of a balanced diet. Balanced means a good variety of foods. Ensure comes in a can, but if needed, it will increase one's weight.

Why not gaining lbs?: A rule of thumb--to gain 1 pound eat 3000 calories more than your body normally burns.  A bigger question is why are you having difficulty gaining weight?  Have you explored with your doctor the possibility of undernutrition,  hyperthyroidism, malabsorption and maldigestion, infection, altered GI tract function, eating disorders, inflammatory conditions, and malignancy?  Pursue evaluation please.

Why gain weight?: Ensure plus is for elderly that don't eat enough.  What's making you think that you need to gain weight? Your bmi is normal. Why don't you increase good nutritional caloric intake by eating real food?

: Probably you have written instructions about what you can and cannot eat and drink in the 1-2 days or so before the procedure. Usually only clear liquids are permitted, which does not include Ensure (unless they have a clear product that I don't know). Follow your instructions carefully:  you do not want to get half way through a colonoscopy that cannot be completed and must be done again!

Dysphagia: The more important question here is why are you having trouble swallowing? The vast majority of causes of dysphagia in a 29-year-old are easily treatable. You should see a gastroenterologist for further evaluation. Do not delay. Good luck.
