Good question!: Some years back this was called a Luteinized Enraptured Follicle (LUF) and was estimated to happen about 1 in 20 cycles. Very little if any research has been done on this recently (to my knowledge), so the answer is "yes, but it's not very common." Good wishes.

Varies: It is possible to ovulate very soon after removing the Mirena (levonorgestrel).  This is partially due to the fact that many women still ovulate with a Mirena (levonorgestrel) in place. I always warn my patients to use backup contraception as soon as the Mirena (levonorgestrel) is out and while they are waiting for their period to regulate.

i had surgery 2 weeks ago, i had 1ovary removed, endometriosis, a lot of scarred tissues, can't concieve without vitro is it poss to have ovulate symptoms?

Yes: Endometriosis decreases egg quality and increases pelvic adhesions. Surgery for endometriosis is notorious to decrease egg number (you lost an ovary). All of these together make it very difficult to conceive naturally in your situation. Because you have your other ovary, you should still be able to ovulate and therefore may experience ovulatory symptoms.

if i miss only 1 birth control pill during the 1st week, take all of the 2nd week, will i ovulate during that time? i missed one pill due to surgery.

BCP: Usually if u miss one pill, you should take the make up pill as soon as you remember and continue on with your cycle. It is unlikely that you will ovulate, but I would use back up birth control this cycle to be on the safe side.

Ovulation tests: I noticed you have asked this question a few times in various ways. Home ovulation tests are to be stopped after a positive result as the LH hormone lingers around. Also these are good screening tests but not definitive tests for ovulation. A luteal phase progesterone is the more definitive way to test for ovulation.

my lh surges are over 8 days long for the past 6 months with regular 29 day cycles. what could cause this, and how do i know when i am ovulating?

I am skeptical: that your LH surges are actually 8 days long if your cycles are regular. The OPK you are using is either misleading you or may be used incorrectly. The OPK is not intended to be used each day once you detect a surge, so I'd suggest strongly that after you get a + you stop using the OPK, it is not necessary or useful to keep testing. 24-48 hours: The usual time of ovulation is 24-48 hours after the + surge is seen.  The surge may show positive for 1-3 days.  If you catch it on the upswing it could be 72 hours later.

28-30 day cycle. took clearblue digital ovulation test (not advanced), & had lh surge detected on day 8 and 11. why multiple days? & so early?

Variable: Ovulation tests must be done daily at the sam time and with making sure that you have the same urine concentration. A more concentrated urine will give you false positive ovulation test. so, please repeat it again daily till Day 16 or till it is consistently positive. In a 28 to 30 day cycle, you are most likely to ovulate Day 14 to 16. Please start testing only from Day 12 of your cycle next time

i have had a positive lh surge from ovulation tests for 5 days. is this normal? if not, what could the reason be? cycle are typically 50 days.

OPK: The accuracy of OPK varies - sadly, the OPK are more reliable the more regular your cycles are normal, meaning the women who need them most will get the least use out of them. 5 days of "lh surge" is not physiologic, so it's either a bad test or you have a very unusual ovulation.
