7-10 days: It can last from 5-10 days and not unusual for pain to move around ..If it persists you might need a cxr.

Cough duration: If the cough is resolving each day and has been present for less than three weeks, continue to drink fluids, monitor your temperature and rest. Nasal saline rinses may be used to reduce post nasal drip.

Days to months: Depending what type of infection it is and whether or not you receive appropriate treatment. A simple vrial infection/cold may last a few days; a wk or so for viral bronchitis and flu (cough may last 3 wks). The so called 'walking penumonia' and pertussis may linger for wks to months if not properly treated. Smokers/cystic fibrosis people may have chronic bronchitis. Consult doc. Good luck.

Rule out meningitis: If you have an infection of the respiratory tract, it may spread to other areas. If you can not move your neck, please have meningitis ruled out by your local ED. Other causes may be related to a muscle strain.

Pretty healthy kid: The average kid is expected to get 5-7 upper respiratory infections per year of varying intensity to the age of 5 when it drops to about  3-5. The above kid sounds pretty healthy.

Infection or ?: a single white spot could be normal secretions that have formed a little seed of material, and are now stuck in the crypts in a normal tonsil.  
Multiple spots, along with a fever and pain, are often a response to a virus.
Blotches of white to green goop that doesnt dislodge with gargling could be a sign of pathogenic bacteria, or (if you are immunosuppressed or taking steroids) monelia fungus.

Cough in winter: If you get these symptoms only during the winter months it may be triggered by allergies and would recommend you have allergy testing done to see it is something you can avoid or get treatment for.  I would also recommend you get your flu shot in late September annually.

Yes!: Blood oxygen level of 92% is likely at rest and when you walk it should go further down, limiting your activities. The poor appetite relates to infection and also to low oxygen level. Insurance does not pay for oxygen supply until you have 88% or less, although you will be short of breath with effort due to lung issues. Continue close care with your doctor to see resolution of this problem.
