Gain weight: At your age it is likely that your mother has all of the necessary information from your earlier clinic visits. That will tell if your weight is OK or not and a decision can be made for you to see the Gynecologist.
Please do not take large amounts of ensure and other drinks till you see the doctor.

See below: Can't say what's normal.  However, you lose weight by watching calories and not by exercise which will only minimally help (exercise is important for health but far less than than calories specifically for weight loss).

Yes.   The first: SSRI, Prozac, came out around 1986 and was thought to be a good alternative to the older antidepressants (Tricyclics), which had terrible reputations about adding extra pounds.   However, lots of people on Prozac gained weight, too, prompting the development of other SSRI's (next came Zoloft, (sertraline) and so on).   Unfortunately, there is no SSRI that will absolutely not put weight on you.   Ask your MD...

Eat healthy foods: high in calories wi 6 meals /day. Use strength training to build large muscle groups. Work out w heavier weights w fewer repetitions. Sleep at least 8- 9 hrs/night. Hydrate well w water. Foods to help w weight gain: almonds, cashews, pecans, sunflower seeds, flax seeds, walnuts, hummus, avocados, granola, trail mix, dried fruits, protein shakes &amp; bars, lean meats, poultry, fish &amp; smoothies.

Weight loss surgery.: Weight Loss surgery would be an option for you.  You'd need to meet with your psychologist or psychiatrist to see if mentally you'd be an appropriate candidate for such surgery.

Lexapro is a medication for primarily for treatment of Depression. , It may cause weight gain.
Benzodiazepines are frequently used for treatment of Anxiety, they are effective and they do not cause weight gain.

It depends: This all depends on your metabolism, how many calories you take in, how many calories you expend.  Weight gain/loss can also be affected by side effects of haart medications, as some cause diarrhea and I am not sure if you are taking any of those.  Talking with your doctor would be important to determine whether nutritional supplements, like ensure, or medications to stimulate weight gain may help.
