Butrans: Butrans takes 3 days to reach adequate plasma levels. I would say if you are not getting any relief by one week then the dose has to be adjusted.

Butrans patch: This is designed to stay on for 7 days. If its falling off then you can use tape to keep it on. It would make no sense to keep it on longer than 7 days - the design provide for steady drug for 7 days only - therafter it may not deliver the same amount.

Potentially a month: or even more.  Buprenorphine is a very long-acting drug, and its major metabolite is also very long-acting. It is bound to serum proteins and both the parent drug and its metabolites can be detected for weeks after chronic use. Eventually only the metabolites are detected, but depending on the sensitivity of the method used for testing, it can be detected for more than a month out.

Careful: There are some adverse interactions using this combination of medications.  This can be allowed for by adjusting doses.  Talk to your prescribing doctor about the potential risks and benefits.

Talk to MD, RPh: prescribers SHOULD know that pain patches may take 24-36 hours to get to levels that may relieve pain, and they also should advise you as to what you can take while the medication is kicking in. Usually, a low dose of a short-acting rapidly absorbed opiate like hydrocodone is reasonable for that short period of time, but I do not know your history, what other meds you are on etc., so can't advise.

my husband has arachnoiditis. the butrans patch didn't work. it is now being treated with 10 mg oxy and 30 mg oxy. barely livable. what else will help?

Avoid surgery, : Which perhaps resulted in postop scarring and arachoiditis, and also do not do steroid injections.  Might consider dorsal column stimulator, and might try some of the nerve pain suppressor meds, such as lyrica, gabapentin, cymbalta, savella, (milnacipran) tricyclics with stelazine.  Sometimes, fentanyl patch of value, but very potent narcotic!

can i drink alcohol while on butrans patch. i have a special occasion over the weekend.  or can i take it off a couple days and be safe?

No, you can't safely: These are serious medications and it is generally not safe to start and stop them for convenience. It is also not safe to drink while on it - If this is for 3/7 or 3/8, talk to the Dr. Don't just remove the patch. Even if it was OK this time, the future may not be. Be careful w/ yourself - that's part of what you need for recovery!

Treat the illness: Rarely there are serious spine conditions that don't have adequate treatment.  Then see a pain clinic. More commonly chronic back pain conditions have more to do with stress, and drug addiction is a common component.  If you have a defined spine condition, fix the problem.  If you have multi-centric pain conditions and nothing seems to work but opiates then determine what you are really treating.

Depends: The answer depends on which nerve is affected and what the underlying reason for the pain syndrome. It is possible to have an increase initially after the procedure, followed by a tapering of the pain. You may also experience new sensations (burning, itching, tingling) transiently. If it works well, the pain sensations from the nerve will go away completely.
