Treat the illness: Rarely there are serious spine conditions that don't have adequate treatment. Then see a pain clinic. More commonly chronic back pain conditions have more to do with stress, and drug addiction is a common component. If you have a defined spine condition, fix the problem. If you have multi-centric pain conditions and nothing seems to work but opiates then determine what you are really treating.
