Depends: It all depends on what has been injured and  how badly you were injured.  In general, it would take a few weeks but if you have torn something, that may take much longer if at all.  If you pain fails to improve signficantly after 2 weeks, consult your doctor again.

Depends on cause: Improvement of symptoms depends on what is the cause.  Muscle spasms respond well and quickly to physical therapy and anti- inflammatories.  Fractures and nerve impingement takes a longer time.  Average for pain is 4-6 weeks.

It : It sounds like you have been through a lot and you have a very complicated medical history.  Spine injuries can take a very long time to heal as you know, especially if you have other medical problems.  It sounds like there would also be concern for you to have surgery as you have known heart problems so would be a high surgical risk.  I would ask that you see a pain management doctor, or a physical medicine & rehab doctor for their advice.  I assume you have seen a neurosurgeon, if not you must have a neurosurgical evaluation.  If you had worsening pain, weakness, or any change in bowel or bladder control then you would have to be seen as an emergency.

Rest heat OTC meda: Generally most just get better with activity restriction and rest but one can apply heat or cold depending what feels best and take over the counter meds like tylenol, (acetaminophen) Naproxen or Ibuprofen if no medical conditions that preclude their use or allergy problems. Guided exercise after a short period of rest is helpful as well.

See below: Rest, ice (within the first 48 hours), Ibuprofen for pain and swelling, then heat after 48 hours), gentle massage and pt if needed.

Talk to your doctor : In general the opiate analgesics lower the seizure threshold making it more likely to have a seizure. Talk to your doctor about your medication. Your particular history will determine whether or not you should take this medicine.  You may want to try non opiates first. Conservative treatment such as rest and NSAIDs are best for back pain.

Depends on injury: Type and severity ax well as how in shape you are to begin with ax well as your overall health.  If just a muscle strain, 2 to 6 weeks on average while a disc issue from a couple of weeks to several months for most.  This is assuming  there is no injury involving a fracture.

A few days: Prednisone is our strongest ant-inflammatory and back pain is inflammatory  if you are not better after a few days of treatment you may need to consider other treatment such as physical therapy or localized facet blocks.
