Completely variable!: Depends on the cause and mechanism of the irregular heart beat. May last seconds to days or more.  Plus depends on one's sensitivity, i.e., some patients feel every irregular beat, some don't feel any even if sustained.  Some causes are intrinsic (diseases) or extrinsic (caffeine, stimulant medications, hormonal, psychological/stress).  The latter causes often last shorter if avoided.

for the past year i have had episodes, heart palpitations, usually last about a minute. have gone to cardiologist, had a stress ekg and nothing shows up.  its irregular. all 4 blood tests each time and all normal? could it be anxiety attacks?

Serious: These cardiovascular symptoms are serious and should be reported to your treating physician as soon as possible. Both palpitations and hypotension have been reported as side effects of nortriptyline. You may be referred for thyroid tests and to a cardiologist to determine the exact nature of your palpitations and for follow-up of your low blood pressure.

i have pain in lower back across the bottom it's the same as when i have heart palpitations but in my lower back lasts about a minute and very intense?

Not enough info: In a time of Too Much Information,  this case is just the opposite.  You need to see family doctor so s'he can get a full history from you and do a thorough physical exam.  You may also need an x-ray and/or some lab tests (including urinalysis.  I wish you the best!

Secs to days: seconds to days, depends upon what is going on.  technically the definition of palpitations is "awareness of the heart beat".  often due to a "not normal" heart beat or arrhythmia of some sort, but sometimes general anxiety cause heighten awareness of the heart beat.

Heart palpitations: Time to see your doctor for a check up concerning the heart palpitations.  If you are having diarrhea, perhaps you are have electrolyte disturbance that could cause palpitations.

Palpitations: There are numerous causes for palpitations. Some are not dangerous, some are of concern.  Many can cause similar types of symptoms.
The best way to figure this out is by wearing a monitor for a period of time. For people with symptoms every day or 2, a 24-hour or 48 hour Holter monitor would work well. For more sporadic symptoms- a longer monitor such as a 2-week Zio or a 4-week event monitor.

Palpitations: Palpitations have many causes from anxiety through anatomic and physiologic abnormalities. The feeling of bounding usually relates to the volume of blood pumped with each palpitation. There is no simple single answer for the question. If you're bothered by the feeling should consult a dr. To see if the problem can be clarified.
