Be happy!: At least a serious problem such as avn or significant osteoarthritis exists.  Your pain may be coming from trochanteric bursitis or your tensa fascia latus. See someone in orthopedics, physical medicine or rheumatology!

Indirectly : Referred pain from hips typically occur in groin or down thigh. That beig said any referral pattern is possible. A possibility may also be your hip avn has led to a dysfunction walking/gait pattern leading to abnormal stresses on back/sacroiliac regions which may cause pain. Get formal eval by a specialist who manages back pain.

Several causes: Whether your medications are contributing depends on what you are taking, and what conditions you are taking them for.  Avascular necrosis (avn) can be caused by trauma, by medications such as prednisone, by chronic alcoholism, and by many illnesses such as lupus or sickle cell anemia.  Your doctor can help you determine what may have caused the avn.

Fall or trauma: Avascular necrosis is typically caused by a very hard traumatic blow to an area or repetitive trauma to an area that causes a lack of blood flow to the bone and therby creating the avascular necrosis.  You need to see your orthopedist.

see no relationship: Avn can be spontaneous, but some common cayses are stearoid use, alcohol abuse, prior dislocation tho this a too far in the distant past, and dialysis.

Stearoids used for any problem can be suspect. Such as for copd, asthma, ra, .

There : There are many causes of avascular necrosis of the hips.  It is generally considered safe to inject in the joint as there it has less systemic effects.  Steroids can cause avn however it is usually attributed to long term use of steroids by mouth.  Other causes of avn include chronic alcohol use, blood disorders, and about a 1/3 that have no known risk factors.

Not likely!: There are no associated reports of this linkage.  Both are not uncommon and coincidently may occur together. Avascular necrosis is a severe form of arthritis of the hip caused by disruption of the blood supply to the cartilage on top of the hip.  Ultimately leading to hip replacement.  Stop gap procedures are in my opinion, ineffectual.

Non-impact: Type and degree of exercise depends on the stage of avn. Usually light exercise (range of motion) in the water and light water aerobics are ok.

Hip replacement: I don't think there is a "right" answer  - depends on your level of pain.  You might want to wait until your other hip heals.     Discuss this with your surgeon.

Depends on severity: Avn pain may be due to early disease, collapse of head, loose bodies or subchondral fractures.  Pain relief arises from pathology specific treatment.  For example, core decompression in early disease vs total hip replacement in advanced disease.
