This is called orthostatic hypotension. Bending over with COPD changes the way blood and oxygen are exchanged in the lungs, like lying flat on one's back. That is called orthopnea. Breathing improves by propping up with pillows.

: Severe COPD can cause secondary erythrocytosis due to chronic hypoxemia. I do not have your medical history and lab results for an appropriate advice but primary erythrocytosis (high Hb/Hct) rarely causes your described symptoms.
