Coronary changes: The heart makes a number of changes. As blockages increase in size, the coronary arteries will increase in diameter in an attempt to continue adequate blood delivery. Unfortunately, this only works to a point and after initial compensaton the arteries can dilate no further and the plaque burden becomes increasingly obstructive. The muscle may respond by contracting less vigourously; hibernating.

Do you mean brain?: Might need to ask a neurologist this one.  Small vessel changes on the brain may or may not be significant for the possibility of vascular dementia but may also be age related.  Best to seek medical evaluation here and undergo some sort of memory assessment.  This will help work out whether there is an issue or not.  Your GP should be able to start the ball rolling here with a memory test

Hard to know: Radiologists are trained to report on images without knowing anything about a patient.  This way, everything gets included; even findings that are normal for most people.  For a person with family history of early onset dementia and cardiovascular disease, for example, these findings could be meaningful.  You can consider a virtual c/s w/ neurologist to review the images and in context.

: The words "chronic" &amp; "small vessel ischemia" imply poor blood flow leading to poor nutrition of the tissue leading to tissue changes. It does not suggest a stroke.

Depends: You can think of angina as similar to the muscle "burn" you feel after prolonged exercise. When the amount of blood flow is not enough to clear the "lactic acid" that is produced with muscular activity there is a burning sensation.  If the insufficient blood flow (and oxygen) remains there can be tissue damage and even death. If you are experiencing angina you need to get checked asap.

Ischemia: Ischemia means inadequate blood flow. This can happen because the large size blood vessels are narrowed. It can also happen with normal large vessels and abnormal small blood vessels causing the inadequate flow. This is 'small vessel ischemia'.  We don't have surgery or stents for 'small vessels' at this time.

General health: Xarelto is pretty effective in preventing strokes but “mini strokes” that cause these little areas can still occur.   If you have high blood pressure it must be controlled; definitely do NOT smoke; get regular aerobic exercise; eat a diet high in antioxidants (fruits &amp; vegetables especially) &amp; keep brain exercising with reading/word puzzles/Sodoku/bridge/etc

i just had a ct scan and it said i have mild bifrontal small vessel ischemia. i have alzheimers in my family - what does this mean?

Many possibilities: My guess is that it may be related to chronic high blood pressure, but it's best to speak with your primary care physician about these results.
