The cause of the fistula may be due to chronic  appendicitis or other factor. It is unlikely to heal on its own .  Infection potentially could develop which could be quite serious

Check With Doctor: The surgeon (or a nurse in the office) should be able  to answer this. Generally, the doctor will want to see you for follow-up after most any surgery anyway. The surgeon knows what s/he had to do to correct the fistula and can best judge how long it should be until  full healing occurs.

: I assume you have a vesicovaginal fistuta. If that is the case, the fistula is transiently obstructed so you do not experience wetting (no leak in the vagina) and bladder can be full then you have the voiding urge. Unfortunately, the fistula will recanalize sooner or later.

when you sit too much and your perianal fistula starts to hurt for days what exactly is happening? is it getting infected or it is just a the wound starts to hurt? and how to response to the pain? immediate medicine or wait for it to heal by itself?

: Sitting can put pressure on the area causing discomfort. These fistula generally require treatment otherwise there are recurrent symptoms. For pain you can try sitting on a pillow, sitz baths and avoiding hard stools. Good luck

Pilonidal cyst: It sounds like a pilonidal cyst if no to minimal symptoms, surgery not needed. Sitz baths may help.  Depending on the doctors expertise, a procedure can be done in the office to fix the problem.  If surgery is needed, its an easy one, same day surgery, minimal down time and discomfort.
