However, if at age 19 it is interfering with your ability to urinate, if it is painful, or if it hinders you in other ways then, you should seek treatment. Moderate risk of paraphimosis (tight phimotic penile ring gets trapped behind the glans, return circulation of lymph & blood is cut off & trapped foreskin swells & can become gangrenous), high risk of trapped smegma due to inability to clean glans penis. Moderate risk of balanitis. In creased risk of stds and & penile cancer.

Medication or circ: Phimosis can be treated with steroid and antifungal creams.  If the contracted part of the foreskin does not soften up sufficiently, then circumcision can be considered.

Phimosis effects: Depends upon severity. Difficult to retract foreskin means hygiene problems &amp; difficulty in remocing smegma. Can develop balanitis, inflammation of penile head and/or foreskin. Paraphimosis, when foreskin retracts and can not be pulled back over penile head, is an emergency &amp; requires immediate correction or can result in necrosis/death of foreskin. Needs steroid cream or circumcision.

Can be an emergency: Phimosis refers to narrowing of the foreskin so that it cannot be retracted over the glans penis (head).  Paraphimosis is the state in which a narrowed prepuce, foreskin, is forcibly retracted over the glans and now cannot be un-retracted.  If the prepuce opening is too small, it may block the blood supply to the glans, necessitating emergent surgical correction.  Phimosis - needs circumcision.
