Well, it sounds: As if you may have contracted a skin mite.  You may just have it first.  No worries, permetharin 5% lotion should take care of it, whther it be bed bug, chiggers or scabies.  Only an exam will really tell but, especially if itches more at night, and badly, i would be going with the skin mite angle.  Gd luc.

Many possibilities: Itchy bump could be many things including an insect bite, cyst, acne, skin cancer, foreign body reaction, scar/keloid, benign tumor.  It should be seen by a dermatologist who can give you an accurate diagnosis and possibly recommend a biopsy and appropriate treatment if necessary.

Check With Doctor: The surgeon (or a nurse in the office) should be able  to answer this. Generally, the doctor will want to see you for follow-up after most any surgery anyway. The surgeon knows what s/he had to do to correct the fistula and can best judge how long it should be until  full healing occurs.

From your limited: description, it still sounds like a hernia that is too small for clinical detection at the time of exam.  It could also be a lymph node or a benign tumor such as lipoma.  If your doctor is not worried about it, your should be reassured that it is probably something trivial.

Tdap Booster: I would agree that you should continue to apply antibiotic ointment, and keep the puncture wound clean. IF an infection were to occur, you would begin to notice redness or your skin, warmth, selling, and tenderness usually within 48 hours of the puncture.
If you do not recall when you last has a Tetanus shot/booster, you should get one of these. Tetanus shot is abbreviated as either Td or Tdap
