Stress: High levels of stress and anxiety can increase your heart rate and blood pressure. Over time this can accelerate atherosclerosis and other processes. Some people react worse to stress than others. The combination of stress and anxiety is probably additive. Finding ways to minimize both can make life both more enjoyable and perhaps less likely negatively effect your health.

What is your issue?: Yes, high levels of stress and anxiety can cause muscle tension, spasms, and twitches.  It can also inhibit normal sexual arousal and function.  Not sure of your issue with cremaster muscle.  Have you lost cremaster reflex? Have you experienced impaired ejaculation or sexual arousal?  Are you working in very cold environments? More than stress?: True visual hallucinations usually comes about when there is more than just stress or anxiety. It more often comes about when the brain is impacted by something such as drugs, alcohol and some times by severe depression. Sometimes during high anxiety the mind may play some tricks and distort some reality. 
In any case you should speak with a doctor if you are having any type of halluciination

Not related: Gbm is not caused by stress. If it were, gbm would be a lot more common than it is. Chronic stress, or reactions to it, can cause or be associated with a bunch of other health issues. Learn to destress, preferably without meds, or drugs like alcohol, pot, etc.

The opposite is true: If I'm understanding your question correctly you want to know if RELIEF from anxiety can result in body soreness/tenderness.  Actually, it would be more likely to be the opposite.  Anxiety of a constant or prolonged nature heightens sensitivity of the NOCICEPTIVE receptors in the skin and muscles (pain receptors) and tend to increase painful sensations. Relaxation should reduce pain sensitivity.

Not necessarely: High stress and anxiety can lead to  suddens dath in people with  high risk factrs like obesity, sedentary life style, lack of exercise, diebetes, high  BP, history of previous  HA or stroke. In young healthy person ,High stress can be surprisingly well tolerated
