Unfortunately : Unfortunately I am unable to give you any information on survival rates without knowing more information.  Based on what is in your post, excess tissue on the back of the neck could be a sign of down's syndrome; the hernia depending on where it is carries different risks.
I would like to take this opportunity to use your case as a lesson for yourself/fiance and others. Never leave a doctor's appointment with unanswered questions. You have a right to have all your questions answered and everything explained to you in detail. If the ultrasound tech. Gave you that information you need to call the doctor right away and ask forn an explanation. 
Make an appointment, call on the phone, write a letter. Do whatever you have to do to get answers.

Not much necessary: Once pregnant, monitoring consists of following bhCG and progesterone values for appropriate increases then an ultrasound to verify intrauterine pregnancy viability (heart beat). This is done in addition to the routine prenatal labs, of course.

In : In general, ultrasound technologists have more experience using the ultrasound machine to obtain the best images possible. In general, it is best to have the person with the most experienced obtaining the images, and this is usually the ultrasound technologist. A radiologist or occasionally an obstetrician always analyze every image obtained by the ultrasound technologist. If there are any questions about any of the images, the physician will usually return and image the patient along with the technologist to clarify any of the findings in question. The radiologist or obstetrician uses their clinical background and the appearance of the ultrasound images to identify any areas of concern. The ultrasound technologist and the radiologist form a highly qualified team to ensure that you receive the best service possible.
