Infection: there is wide range of potentials most commonly if infection, other causes include stress (physical or emotional, vigorous exercise), smoking, some medications, autoimmune diseases, other blood or systemic disorders

Probably not: This would be most suggestive of polycythemia vera and at age 22 this isn't a consideration. You may simply be athletic. Reference ranges are set so that several percent of healthy folks fall outside. If you feel well, and your physician didn't make a big deal of it, stop worrying.

The modest left shift (ie., relative increased neutrophil %) could indicate an effort to address a mild bacterial infection. But more likely it's just normal variation. I do wonder what prompted the check to begin with? Additional details?

Possible hepatitis: There are many causes of hives.  Allergic reactions are the most common.  Persistent hives might be from a medication, or an environmental allergen, for example.  Infections can cause hives.  The abnormal alt and ast can be a sign of viral hepatitis. Sometimes hepatitis will cause hives, especially in the early phase, before jaundice sets in.  Chronic hives (more than 6 weeks) is more complex.
