Sepsis: High lactate levels can be from multiple causes. The most common is low blood pressure limiting blood supply to vital organs and tissues. This  can be seen in sepsis. 
Most patients with high lactic acid levels are critically ill and require ICU level of care.

In the setting of?: L:P ratio &gt; 30 suggests inherited disorders of the respiratory chain complex or tricarboxylic acid cycle disorders. An elevated lactate and a L:P ratio less &lt; 25 suggests a defect in pyruvate metabolism. Are you being worked up for congenital lactic acidosis?

Don't stop: So, Lactic Acid builds up from working out - exercising.  If you stop rather than go through a cool down period - it will build up and cause pain.  You need to - if a runner - go to a slow run/jog/walk at the end of your run - to continue good blood flow and removal of the lactic acid.

No: Elevated lactic acid levels suggest heart. liver or lung problems (can also be associated with signif infection or anemia).  Levels are up when non-aerobic respiration occurring in body. Small amount: Lactic acid build up can occur from a variety of causes. Most times this is a limited disturbance due to a severe illness in hospitalized patients (sepsis or liver failure). It can also occur during aggressive exercise, but is generally never severe enough to require medical attention.

Needs to be treated: Lactic acid is produced by tissues/ cells that are not getting enough oxygen.  The most common cause is due to patient's blood pressure being so low that organs such bowel/ kidneys/ brain are not getting enough oxygen.  This always has to be treated, as it can be life threatening.  Cause of low blood pressure needs to investigated, however quite often is due to an infection causing severe sepsis.

No: Lactic acid, although we say "a build up of lactic acid" , etc, does not make a bump or knob. You could have a muscle spasm of some sorts or another injury such as a muscle tear.
