Many causes: Anything from an infection to a serious blood disease to just a little bit of stress. I can triple my granulocyte count with a 20-minute cold shower as my neutrophils leave the walls of my blood vessels and enter my bloodstream to see what's happening. If the neutrophils are immature (left-shift) or look very activated (toxic granulation) there's likely to be an important infection.

Need numbers: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex. 
Get HPV vaccine.

Blood test: To place your test results in a better perspective, discuss them with your provider and/or see a hematologist. Avoid the guessing game.

Not yet: Percentages of lymphocytes and granulocytes are meaningless apart from the absolute values. My RDW runs about like yours when i donate blood, i never understood the need to measure platelet volume routinely... If your physician isn't worried, this pathologist ("look at the person, not the labs") isn't either.

Diabetes? Other(s)?: Can't help much with only this information. Elevated glucose suggests diabetes. Otherwise, such test results could indicate dozens of problems, perhaps no single illness. The likely explations will depend on symptoms, physical exam, and probably X-rays or any number of other tests. Discuss with your doctor.

No: White cell percentages mean nothing and I wsj they were not even reported. If the absolute values were anywhere near reference range, and all the cells looked okay, you are fine.

Find the cause of in: You need to have your doctor check you in order to find the cause of Infections. It is not normal to have infections so frequently as you have indicated. In the meantime, get adequate sleep/rest at night and eat healthy foods(more of fruits and veggies and less meats) and find sometime to exercise regularly. That will give you a stronger and healthier Body.

WBC counts: absolute granulocyte count=total WBC count x % granulocytes
This is a means of monitoring the risk of infection in people getting chemotherapy mostly, as a low absolute count are high risk for infection. It is more useful than either the total WBC or differential percentage alone.

Abnormal blood count: I would recommend a repeat blood count. If it is still abnormal, I recommend seeing your health care provider.

It depends: The WBC count normally has mature granulocytic blood cells. If they are immature then we need to know how immature and how many there are(%age). Infections can cause immature granulocytes to show up in blood for a short time period and then normalize. I would ask you to repeat your CBC and WBC and differential count. If the abnormaility persists more than a couple of weeks, see a speicialist.
