Probably no: IN general, a dose is not too high if it is needed for a full effect, and it is not causing side effects.  More specifically, 15 mg escitalopram and 45 mg mirtazap;ine are within a standard range.

No 'Gold' standard: These drugs in combo, often for 'off-lable' symptoms (GAD) are a matter of trial-and-error and each Dr's clinical experience. If patients give accurate feedback, some become good at using them. Others may just try different doses and combos. Discuss it together and ask all you want. How do you feel on this dose?

i've been on 75mg of mirtazapine for 7 months , primarily for anxiety and insomnia. is this high dose normal? how would you reccommend cutting back?

High dose.: Does seem like a large dose, but may be appropriate for you.  Personally i would prefer to see you on a "cocktail" of a few meds at lower doses.  As far as tapering could reduce by 1 tab every week .  Would not do this without discussing with your doc and having a plan for addressing symptoms if they occur.

i have anxiety and insomnia from very high estrogen level from hrt. dr. prescribed mirtazapine for anxiety and sleep. i've used zzquil for several yrs at a standard dose. can i stop the zzquil w/o withdrawal if mirtazapine helps with my insomnia?

: If high estrogen level is the culprit, dose should be reduced not treating its side effects with another medication. I am not sure why you need HRT but a high estrogen level is not recommended at your age. Otherwise, I doubt if you will get serious withdrawal symptoms with stopping the OTC sleeping formula with or without mirtazapine.

Remeron (mirtazapine): Remeron (mirtazapine) has been reported to raise both cholesterol and triglycerides. I advise speaking with your pharmacist  and psychiatrist who may have more knowledge about this and other antidepressants.

0: Usually the maximum dose is 45 mg, however in the elderly we start low and go slow. We start at as low as 7.5 mg and go upto 15 or 30 mg depending upon what it is being prescribed for.
