Question cut off but: Dysphagia is distinguished as "transfer dysphagia" (problem moving food from mouth to top of esophagus) versus "transit dysphagia" (problem getting food down esophagus &amp; into stomach). Motility disorders, strictures, ulceration (pill-associated, viral), tumors, eosinophilic esophagitis, spasms, etc must all be considered.

See a doc!: You may just have some swelling causing the tightness, but it's best to seen an ENT to examine the area. Treacheal fractures and other serious injuries are rare and you'd likely have significant symptoms, but you should have this evaluated. You may have a hematoma or some other issue that may need intervention.

Pulse: It may be trivial or serious, it can be anxiety but can also be something with the heart. At any rate, i think you should get checked to reassure you and / or to get some tests done.

Vascular disease?: One possibility is carotidodynia, a painful disorder involving either carotid artery. Cause often unknown. Other similar causes include arteritis, arterial dissection. Chest involvement could indicate involvement of arteries closer to the heart. Disorders involving the base of the heart (which is actually at the top) can also do this. Consider seeing cardiologist or neurologist.

Very well could be: Is the insomnia and early morning awakenings coming from sharp earaches by any chance? You will need a good examination by your physician who will decide whether or not an endoscopy or other procedure are merited to find out what's up.

Hiatal hernia: Yes the hernia can give tightness, fullness and discomfort especially after eating. Try small frequent meals and sitting up after eating.

Gastroenterology: Ct is great for seeing the abdominal organs but not the best at examining the lining of the stomach or intestine in conditions like gastritis.  I would recommend consulting with a gastroenterologist.

Swallowing problem: Is it rare and associated with heartburn or more consistent? It may be minor spasm or can be more serious. A GI specialist can help you find out best. It helps to study it to eliminate worry.

Infection: Infection of the throat (sore throat) or tonsils.  If it persists, go see a doctor.
