See your doctor: Your fissure may not be healing properly.  Sometimes they form a fistula which can compromise bowel and bladder function.

Anal fissures are: most common, annoying painful condition , which will take long time to heal . Drink plenty of fluid , eat high fibre diet , and avoid constipation , use stool softeners   most of the time will heal with out  surgery. For rash you may need local care and  Rx antibiotic cream  see your doctor .

Comfort: Usually I do not recommend epsom salts. Just comfortably warm water is usually sufficient. Ask your own surgeon for instructions.

Rectal pain: You are probably right.  Typically this could be hemorrhoids.  An anal fissure is certainly possible.

In any event, softening your stool with dioctylsulfosuccinate and using lubrication like vaseline or aquaphor should help

Yes but don't buy it: There are many herbal "treatments" on the internet that will take your money.  Truth is most early fissures will heal on their own if you ensure that your bms are soft (fluid + fiber) and soak after bms and give it some time.  Chronic fissures benefit from any med that paralyzes the anal muscle and relieves the spasm caused by the painful fissure.  Rarely do you need surgery for a stubborn fissure.

i think i have internal hemorrhoid but could be anal fissure. my stools clearly show "gouging" caused when passing through the rectum. suggestions?

Fissure is painful: Anal fissure is very painful mostly and internal hemorrhoids are painless on the contrary, a thorough evaluation by a physician preferably a colorectal surgeon is recommended to make sure no serious pathology is going in.

: General surgeon. In the meantime increase your intake of fiber and water to keep your stool soft. You may take a fiber laxative like Metamucil and drink enough water daily so that your urine is colorless. For the long term you may try to increase your intake of fiber foods.

You need a correct diagnosis: An anal fissure and hemorrhoids are two entirely different things. You need to get the correct diagnosis because the treatment of these issues are entirely different. Go get another opinion. Are you sure you saw a physician?
