Diet & exercise: Make sure you exercise at least 30 minutes a day, don't skip meals, avoid soda, juice, dairy, alcohol and fried foods.  Eat a healthy well balanced diet! also water, organic vegetable broth, green leafy vegetables, natural pickles, apples are great to fill you up so you dont feel hungry and they are very low in calories.
Get at least 8 hours of sleep at night.

Hemorrhoids: Hemorrhoids are best treated with stool softeners, sitz baths and increasing fiber in the diet.  If these do not work, then see your primary care physician.

For cure: Only surgery or banding.  But symptomatically, things like preparation h or witch hazel help.  Also stool softeners and diet rich in fruits and veggies.

Case by case: This would be decided on a case-by-case basis.  Your surgeon consultant would be able to recommend or not, after speaking with and examining you.  If hemorrhoids are thrombosed (clotted) and painful though, surgery might well be recommended.

Proctofoam HC: Proctofoam/ Anusol HC twice a day; Stool softener, Hydration, Sietzs bath will cure and prevent recurrence. If liver disease.....causing portal hypertension....need to be treated.

Tratamiento: No hay un solo mejor tratamiento sino que depende del paciente y de su condicion. La combinacion de antidepresivos, calmantes del dolor y medicinas para dormir parecen dar buenos resultados. There is not only one best treatment for these conditions but it depends on the patient and  individual presentation. Combinations of antidepressants, pain relievers and sleep medications seem to be effective.

Before treatment...: Could you be referring to fissures, fistula, abscess? Sexually transmitted disease (syphilis, gc, hiv, etc.) should be considered. Hemorrhoids ; rectal prolapse can appear as an outward bulging. Anal trauma of any kind may cause mechanical injury. Skin eruptions need to be considered, as does "pruritis ani" (see my healthtap answers on the latter subject). Go get checked for appropriate diagnosis.
