Can be either, both: Eating a lot can lead to stomach reflux wakenings during sleep. Anxious people are too aware of normal sensations a human experiences 24 hours a day. Normal people's brains filter out most normal sensations and only focus on abnormal sensations. For example, everyone has pulsations with heart beats. Most of us don't feel the normal pulsations, but anxious people may obsessively focus on them.

PVC possibly: Several things come to mind, but you almost exactly describe the feeling some patients get when they have a premature beat (these are typically benign), the beat after a premature beat typically feels like a jolt.  Other reasons for this type of a feeling could be OSA and related sleep disordered breathing maladies.

You would have to wear an outpatient holter monitor to determine whether the premature beats are atrial, ventricular or av nodal. It would also tell you how many premature beats occur in 24  hours, ?runs&amp; when they are most prevalent, Discuss with your MD

Supraventricular tachycardia, atrial fibrillation/flutter, ventricular tachycardia all can cause a rapid pulse.  Sometimes just the normal rhythm going faster than normal may cause a sensation of palpitations.
