Yes, they could: but would be better off not smoking, but getting an inhaler. They make the equivalent of electronic cigarettes for use with marijuana, where there is a heating element that volatilizes the THC from the plant, without producing smoke, carbon monoxide and other harmful elements. However, marijuana increases heart rate, which is often not good for people with underlying heart disease. Talk to your MD

Very bad: Coronary artery disease and peripheral artery  disease all worse and even can cause sudden death in active smokers. Stop smoking before is too late. Consult your physician for in depth conversation.

Hyperlipidemia: There is a high correlation between smoking, drinking and increased LDL cholesterol with another heart issue.  In patients with with previous heart disease, an elevated LDL is correlated with a greater than 20% chance of another heart problem.  There is a significant risk of both drinking and smoking in the worsening of heart disease.  You do all 3 and the risks are probably additive.

Nicotine : Nicotine increases Insulin resistance. This in turns can lead to higher sugar levels. . People with pre-diabetes or diabetes  are at an increased risk of developing cardiovascular disease.

Any exercise: For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex.
