i have constant heart palpitation for 12 months.it started one day after celebrating new year.i am 22 years.what could cause constant palpitation?

Palpitation: You have anxiety  and taking lorazepam. Second sinus problem and hope you are not taking sudafed or sinus meds. Third there are many other problems  such as heavy caffeine intake, recreational drug use, thyroid disease, heart ; valvular disease. Skipped beats: In a young person like yourself it's probably fine. Cut down on caffeine (yea soda energy drinks coffee) and obviously any drugs if you're using them. If it's associated with passing out or nearly passing out then seek attention now.

Defined: Palpitations are, by definition "an uncomfortable or abnormal awareness of one's own heartbeat." the most common forms of this, which we call pacs or pvcs, are not in fact skipped beats but rather a single extra beat or two which cause a "pause" afterward, making it feel like a skipped beat.

Hard to say: You say you're on a monitor. I am assuming the results are not back yet. If the skip beats and palpitations occur while you are on the monitor then a record is made and your cardiologist should be able to get a better idea of what is going on. Many have ways for you to mark when you have symptoms, too.  An electrophysiologist (heart rhythm specialist) should be able to help.

i have tingling in head, leg and arms. i also occassionaly feel high palpitation and hear beats in my throat.
my hearts feel very heavy next day exercise.

The esophagus passes along the back of the heart so that if it gets irritated it can affect the heart beat. Residents i work with did a study showing EKG changes if cold water is swallowed. There are other possible mechanisms for palpitations as well.
